Thank you for choosing The Geelong Clinic for your patient's mental health needs.
Please follow the link below to login to our eReferrals portal which will allow you to submit an online one page referral. Once your referral has been submitted, the team at The Geelong Clinic will be in contact with your patient to arrange an assessment. Your account also allows you to view the status of your submitted referrals.
To assist you with this process, please have the following patient information at hand:
- Patient Name and DOB
- Diagnosis details
- Referring doctor, requested admission/transfer date, requested assessment date
- Funding details (eg. Private Health Insurance, DVA etc)
Important. If you are registering as a new user, you will also require a valid email address and contact phone number to submit referrals online.
